The Cigna Group deploys AI through what it calls a health intelligence engine: its own medical, pharmacy and behavioral data and predictive models, combined with language models from OpenAI and Anthropic and service agents from Sierra. Chief Data, Digital and AI Officer Katya Andresen owns the strategy, and AI runs in Cigna Healthcare's myCigna assistant and care management and in Evernorth's Accredo specialty pharmacy. The Cigna Group has published no business return for AI, and its $3 billion Accelerate to One savings plan (Sep 2026) does not give AI's share.

Their approach to AI

The platform
The Cigna Group builds its own data assets and predictive models into what it calls a health intelligence engine, and buys foundation models from OpenAI and Anthropic, voice agents from Sierra and workplace tools such as Microsoft Copilot and Cursor (Aug and Sep 2026).
The models
The Cigna Group uses large language models from OpenAI and Anthropic (Aug 2026), began a clinical AI collaboration with OpenAI in Sep 2026, and runs proprietary machine learning models, with a machine learning patent filed as early as 2014.
The reach
The myCigna AI assistant was available to about 10 million Cigna Healthcare customers in Jul 2026; The Cigna Group has not said how many of its 67,700 employees use Microsoft Copilot or Cursor.
The catch
The Cigna Group has published no business-level return for AI; its largest AI savings figures are projections that do not say whether Cigna or its customers keep the money, and it faces litigation alleging improper AI use in claims.

Key findings

  1. The Cigna Group has published no business-level return credited to AI: its AI figures are service measures, such as the 20% drop in phone calls per Cigna Healthcare customer over about two years that Katya Andresen tied to AI in digital channels on Sep 30, 2026, and projections, such as about $200 million in care management savings over three years (Jul 2026).

  2. The Cigna Group launched Accelerate to One at its Sep 30, 2026 investor day, a $3 billion savings program for 2026 to 2030 built on automation, workforce effectiveness and supplier management and enabled by technology and AI, without saying how much of the $3 billion AI will deliver.

  3. The Cigna Group builds its data and predictive models and buys the rest: Katya Andresen said in Aug 2026 that Cigna works with OpenAI and Anthropic on their language models, Sierra's voice agent handles more than 40% of Accredo calls on its own (Sep 2026), and a clinical collaboration with OpenAI announced on Sep 23, 2026 starts in oncology.

  4. Evernorth committed $100 million through 2028 to Pharmacy Forward, an AI program at Accredo Specialty Pharmacy launched on Jul 1, 2026 that targets halving the time from prescription to medication and about $400 million in value by the end of 2028.

  5. The Cigna Group approves models through Enterprise Model Governance under six Responsible AI Principles and generative AI use cases through an AI Center of Enablement, moved board oversight of AI to its Finance & Technology Committee in Jan 2026, and disclosed in its 2025 Form 10-K that it faces litigation claiming it improperly used AI in claims evaluation.

Where Cigna sits

Stage
Experimenting

The Cigna Group has reported no business return it credits to AI, for any use case or for the company as a whole. The figures it ties to AI are service and task measures, such as a 20% drop in phone calls per Cigna Healthcare customer over about two years that Katya Andresen tied to AI in Sep 2026, a figure with no cost attached, and projections, such as about $200 million in care management savings over three years (Jul 2026), which do not say whether Cigna or its customers keep the money. The $3 billion Accelerate to One savings target for 2026 to 2030 names AI among several levers and gives no AI share. The Cigna Group has not reported a company-wide AI rollout with a user count either: Chief Data, Digital and AI Officer Katya Andresen owns AI strategy and has launched Microsoft Copilot and the Cursor coding agent, according to Fortune (Aug 2026), but Cigna has not said how many of its about 67,700 employees use them, and the about 10 million with access to the myCigna AI assistant are customers, not staff. AI does run in production in Cigna Healthcare and Evernorth. The Cigna Group stays at Experimenting until it reports one return it credits to AI, or states how many employees use its AI tools.

How they deploy

Build for advantage, partner for capability

The Cigna Group's 2026 investor day set a build-or-partner rule: Cigna owns the health care context, clinical intelligence, personalization and service workflows, and uses outside foundation models, voice platforms, agent frameworks and infrastructure. Brian Evanko named frontier AI models as an area where Cigna partners with companies that can deliver them more effectively than it can (Sep 2026). Katya Andresen told Fortune in Aug 2026 that the mix of Cigna's proprietary data and partnerships with large AI companies will set the company apart.

A health intelligence engine

The Cigna Group describes its AI as a health intelligence engine that combines medical, pharmacy, behavioral and lab data across millions of customers with its clinicians and pharmacists to predict needs, choose the next interaction and personalize outreach. Katya Andresen told investors in Sep 2026 that the engine is an existing platform built up over years, citing more than 700 active and pending patents across the enterprise and a machine learning patent filed in 2014.

Problems first, measured by value

Katya Andresen said at the Sep 2026 investor day that The Cigna Group does not start by asking what to do with AI, and Cigna's deck said it measures value realized over activity and rethinks whole systems over single use cases. Brian Evanko said on the Jul 30, 2026 earnings call that The Cigna Group will focus AI investment on applications that improve affordability, customer experience and clinician effectiveness.

AI augments clinicians

The Cigna Group's 2025 Form 10-K says most of the AI models Cigna uses are augmented intelligence that give information to human experts, and that physicians or employees make the decisions in claims reviews. Chief Medical Officer Amy Flaster told investors in Sep 2026 that The Cigna Group uses AI to augment clinical expertise and never to replace it.

AI as a cost lever

CFO Ann Dennison said in Sep 2026 that The Cigna Group's $3 billion Accelerate to One program will cut costs through 2030 by automating workflows with AI, technology or re-engineering, building a future-ready workforce and managing suppliers more tightly, and that technology and AI will speed up all three.

Deployment model
The Cigna Group runs AI from an enterprise data, digital and AI organization under Katya Andresen, which leads data commercialization, digital, advanced analytics and enterprise AI strategy along with The Lab by Evernorth and the Evernorth Research Institute. Global CIO DP Koka's Technology and Operations organization runs infrastructure, cybersecurity, application development and the customer, client and provider service operations. The Cigna Group ships AI inside its businesses: Cigna Healthcare member service and care management, Evernorth's Accredo specialty pharmacy and MD Live telehealth, and Express Scripts Pharmacy Benefit Services. New generative AI use cases need approval from the AI Center of Enablement, and models pass the Enterprise Model Governance approval process.
Model providers
The Cigna Group works with OpenAI and Anthropic to use their large language models, Katya Andresen told Fortune in Aug 2026. On Sep 23, 2026 The Cigna Group and OpenAI announced a collaboration that brings OpenAI's frontier reasoning models into clinical workflows and consumer experiences, starting with oncology nurses and case managers at Cigna Healthcare and Accredo; The Cigna Group's investor day deck lists Accredo's Therapeutic Resource Centers as the first use, live with a subset of nurses, and Health Sense as the second, due in early 2027. Sierra supplies conversational and voice AI agents for customer service, and The Cigna Group has launched Microsoft Copilot and the Cursor coding agent as workplace tools. The Cigna Group also builds its own predictive and machine learning models for care management and has not published which foundation model versions it runs.
Data and infrastructure
The Cigna Group's health intelligence engine draws on medical, pharmacy, behavioral and lab data across millions of customers, and Evernorth's pharmacy platform adjudicates more than two billion adjusted prescriptions a year (2025 Form 10-K). The Cigna Group's 10-K lists operating data centers among its business functions and refers to third-party cloud service providers without naming them, and Cigna has not said where its OpenAI, Anthropic or Sierra workloads run. The Cigna Group closely controls third-party vendors' access to its data, according to Fortune (Aug 2026), and says patient data in the OpenAI collaboration is handled under privacy and security practices, with clinicians responsible for patient care (TechTarget, Sep 2026).

Platforms and internal tools

  • Health intelligence engineNamed Sep 2026

The health intelligence engine is The Cigna Group's name for the combination of its proprietary data, clinical teams and AI that predicts what is likely to happen to a customer, defines the next interaction and personalizes engagement. The Cigna Group describes it as three capability groups, what it knows, what it predicts and what it does, with each result fed back to sharpen predictions; the engine is a description of existing assets, not a single product.

The Cigna Group has run predictive models for more than a decade, filing a machine learning patent in 2014 and piloting cancer prediction models in 2021, and by Jul 2023 had an AI Center of Enablement approving new AI use cases. Customer reach is the best-documented measure: the myCigna AI assistant, launched in Jun 2025, was available to about 10 million customers in Jul 2026, and more than 1,250 Cigna Healthcare care management clinicians work with AI that prioritizes their outreach. Employee reach is not quantified: Katya Andresen has launched Microsoft Copilot and the Cursor coding agent as workplace tools, the 2026 proxy statement says The Cigna Group invested in AI learning for employees in 2025, and the Sep 2026 investor day cited an AI Institute for learning, but The Cigna Group has not said how many of its 67,700 employees use AI tools.

Spend and ROI

Technology budget

The Cigna Group does not disclose a technology or AI budget. The only AI-specific spend figure it has published is Evernorth's $100 million for Pharmacy Forward through 2028 (Jul 2026), set against revenue of $275 billion in 2025 and 2026 guidance of about $280 billion in adjusted revenue.

Accelerate to One

The Cigna Group launched Accelerate to One on Sep 30, 2026, a $3 billion multi-year savings program for 2026 to 2030 in three areas: automation and process modernization, talent effectiveness, and supplier, partner and vendor management, all enabled by technology and AI. Brian Evanko said the $3 billion is cumulative cost savings by 2030, part reinvested and part supporting earnings targets, and Ann Dennison said technology and AI will speed up the work; The Cigna Group did not say how much of the $3 billion depends on AI.

Reported ROI

The Cigna Group has not reported a business-level return, in revenue, cost, unit cost or headcount, that it credits to AI. The Cigna Group's AI figures are projections or savings for customers: about $200 million in estimated savings over three years from expanding AI-enabled care management, based on about $2,000 lower annual medical costs per engaged customer (Jul 2026); about $400 million in value from Pharmacy Forward by the end of 2028, of a kind Evernorth did not specify; and a Health Sense target of 10% less inefficient care by the end of 2030. Katya Andresen said a biosimilar campaign shaped by AI produced more margin, without a figure, and a couple hundred million dollars of savings for patients (Aug 2026).

Productivity markers

The Cigna Group reports service and task-level gains that it has not converted into cost: Katya Andresen tied a 20% drop in phone calls per Cigna Healthcare customer over about two years to AI in digital channels, AI cut MD Live clinicians' note-taking time by up to 90% and specialty benefit reviews at Express Scripts from 15 minutes to 21 seconds, and a Sierra voice agent handles more than 40% of Accredo calls on its own (Sep 2026). On the Apr 2026 earnings call Brian Evanko credited a 20% drop in inbound calls from digitally eligible U.S. employer customers, and a 25% drop for pharmacy benefit members, to the combination of AI tools, contact centers and improved digital experiences, without separating AI's part. Evernorth targets up to 50% less clinician documentation time and half the time from prescription to medication through Pharmacy Forward.

Headcount

The Cigna Group employed about 67,700 people at the end of 2025, about 88% in the U.S., and Brian Evanko cited more than 60,000 colleagues at the Sep 2026 investor day. The Cigna Group cut about 2,000 jobs, under 3% of its workforce, in Feb 2026 to drive efficiency, without citing AI (Greenwich Time via InsuranceNewsNet). Accelerate to One includes building a future-ready workforce, but The Cigna Group has not tied AI to job cuts and gave no headcount target for the program at the Sep 2026 investor day.

Governance and risk

Model approval

The Cigna Group's Enterprise Model Governance team oversees the development, deployment and monitoring of AI models under six Responsible AI Principles: validity and reliability, safety, privacy, fairness, transparency and accountability. An EMG Board of senior leaders from business, clinical, privacy, legal, internal audit and information protection runs an enterprise-wide approval process for models in use or in development (2025 Form 10-K). Katya Andresen told Fortune in Aug 2026 that Cigna's governance of machine learning models dates back more than a decade.

Generative AI approvals

The Cigna Group's AI Center of Enablement brings together technology, privacy, data governance, security, legal, compliance and marketing staff to evaluate and approve generative AI use cases against the Responsible AI Principles and health care privacy and security rules (2025 Form 10-K). Katya Andresen said in Jul 2026 that Cigna brings cybersecurity, legal, compliance and model oversight experts into AI design from the start.

Board oversight

The Cigna Group's board moved oversight of AI and cybersecurity to its renamed Finance & Technology Committee from Jan 2026, after the Finance Committee and Audit Committee held those duties in 2025; Kathleen Mazzarella chairs the committee from Apr 2026. The committee receives regular AI updates from the Chief Digital & Analytics Officer, the proxy's name for Katya Andresen's role, who also reports to the full board on AI once a year, and cybersecurity updates from the Global Chief Information Security Officer (2026 proxy statement).

Claims decisions and litigation

The Cigna Group's 2025 Form 10-K says people, including physicians, make the decisions in claims reviews and that most of its AI models are augmented intelligence, and discloses that Cigna faces litigation claiming it improperly used AI in claims evaluation. In Kisting-Leung v. Cigna, over the PXDX claim review process, a federal court in California granted in part and denied in part Cigna's motion to dismiss on Mar 31, 2025; an Aug 24, 2026 stipulation set the plaintiffs' class certification brief for Oct 29, 2026 and noted a parallel PxDx class action, Snyder v. The Cigna Group, in Connecticut.

Clinical AI guardrails

The Cigna Group says AI-generated information in its OpenAI collaboration is subject to human oversight, that clinicians remain responsible for patient care, and that outreach data becomes part of the patient's record (TechTarget, Sep 2026). Amy Flaster said in Sep 2026 that Cigna uses AI to augment clinical expertise, never to replace it.

Regulation

The Cigna Group's 2025 Form 10-K lists AI rules from states such as California, Colorado and Texas, FTC enforcement on data and AI, the NAIC's work on AI, and the EU AI Act, and warns that generative AI can produce false or inaccurate output that could bring fines, retraining or limits on Cigna's use of AI.

AI in hiring

The Cigna Group says it uses AI hiring tools from Phenom only with human oversight, that people decide which candidates advance, and that it does not let AI discriminate on protected characteristics or use zip codes as a proxy for them (Mar 2026).

Quick answers

How does The Cigna Group deploy AI?
What does The Cigna Group use AI for?
The Cigna Group has not published a count of its AI use cases; the ones it names run in Cigna Healthcare member service and care management, Evernorth's Accredo specialty pharmacy and MD Live telehealth, Express Scripts' pharmacy benefit reviews, hiring and workplace tools. The Cigna Group has published no business-level return credited to AI for any of them: its AI figures are service and task measures, such as a 20% drop in phone calls per Cigna Healthcare customer over about two years (Sep 2026), and projections, such as about $200 million in care management savings over three years and $400 million in Pharmacy Forward value by 2028, which do not say who keeps the money. The $3 billion Accelerate to One savings target is not broken out by AI.
Who leads AI at The Cigna Group?
AI at The Cigna Group is led by Katya Andresen (Executive Vice President and Chief Data, Digital and AI Officer); DP Koka (Executive Vice President and Global Chief Information Officer). Ownership of AI on the org chart: EVP and Chief Data, Digital and AI Officer, on the Enterprise Leadership Team.
What AI platform does The Cigna Group use?
The Cigna Group builds its own data assets and predictive models into what it calls a health intelligence engine, and buys foundation models from OpenAI and Anthropic, voice agents from Sierra and workplace tools such as Microsoft Copilot and Cursor (Aug and Sep 2026).
Which AI models does The Cigna Group use?
The Cigna Group uses large language models from OpenAI and Anthropic (Aug 2026), began a clinical AI collaboration with OpenAI in Sep 2026, and runs proprietary machine learning models, with a machine learning patent filed as early as 2014.
How many Cigna employees use AI?
The myCigna AI assistant was available to about 10 million Cigna Healthcare customers in Jul 2026; The Cigna Group has not said how many of its 67,700 employees use Microsoft Copilot or Cursor.
How far has AI spread at The Cigna Group?
