UnitedHealth Group
Scaling- Healthcare
- 390,000+ employees
- ~$1.5B planned AI investment (2026)
- United AI Studio, built in-house
- 1,000+ AI tools in production
UnitedHealth Group deploys AI through United AI Studio, an in-house platform that gives its teams hyperscaler models and small language models trained on company data, and says more than 1,000 AI tools run in production across UnitedHealthcare and Optum. UnitedHealth Group plans to invest nearly $1.5 billion in AI in 2026, and UnitedHealthcare expects nearly $1 billion of 2026 operating cost reductions, many of them AI-enabled. Optum Insight, run by Sandeep Dadlani, sells versions of those internal tools to other payers and providers.
unitedhealthgroup.comTheir approach to AI
- The platform
- United AI Studio is UnitedHealth Group's proprietary AI platform for building, deploying and monitoring AI under common practices; Optum Chief AI Officer Shobhit Varshney has overseen it since Jul 2026.
- The models
- UnitedHealth Group gives staff hyperscaler large language models and its own small language models through United AI Studio (May 2025), had 117 large language models available to staff by Jun 2026, and uses Anthropic's Claude (Jul 2026) and Microsoft and Google AI in Optum Real.
- The reach
- More than 80% of UnitedHealthcare member calls used AI tools in Jan 2026, and ambient AI note-taking was available to 70% of employed Optum Health providers in Jul 2026, with more than 90% targeted by the end of 2026.
- The catch
- UnitedHealth Group reports AI's financial effect as targets and an unexplained ratio: nearly $1 billion of 2026 cost cuts at UnitedHealthcare, and a 2-to-1 return executives told Bloomberg they see (Jun 2026). No dollar figure is credited to any single AI tool.

Key findings
UnitedHealth Group says more than 1,000 AI tools run in production across UnitedHealthcare and Optum (Jan 2026), and CEO Stephen Hemsley said on Jul 16, 2026 that every function, HR, finance and legal included, is being reimagined around AI.
UnitedHealth Group plans to invest nearly $1.5 billion in AI in 2026 and at least as much in 2027, about a third of it on turning internal tools into Optum Insight products; Sandeep Dadlani said in Apr 2026 that the company expects a 2:1 return over the next few years, and executives told Bloomberg in Jun 2026 they were already seeing a 2-to-1 return, without saying how it is measured.
UnitedHealthcare expects nearly $1 billion of operating cost reductions in 2026, many of them AI-enabled, and Tim Noel said on Jan 27, 2026 that more than 80% of member calls already use AI tools to answer questions.
UnitedHealth Group spreads AI ownership across several leaders: Shobhit Varshney, hired from Citi, became Optum's Chief AI Officer in Jul 2026 over United AI Studio and AI delivery; Ujjwal Ratan, from AWS, became Optum Insight's chief AI officer in Sep 2026; Michael Pencina joined as chief AI scientist in Oct 2025; and the Chief Legal Officer has owned AI governance since 2024.
UnitedHealth Group's board tied 20% of the 2026-2028 Performance Improvement Awards for senior executives to reportable earnings from AI products sold to outside customers, according to the Apr 2026 proxy statement.

How they deploy
Build the platform, mix bought and in-house models
UnitedHealth Group built United AI Studio as a secure platform that gives employees access to large language models from the major AI hyperscalers and to small language models UnitedHealth Group trained on its own datasets, Sandeep Dadlani told Fortune in May 2025. UnitedHealth Group says United AI Studio sets the practices its AI experts follow at every stage of building, deploying and managing AI applications. Dadlani told Bloomberg in Jun 2026 that staff had 117 large language models available and that 99% of UnitedHealth Group's AI applications are administrative rather than clinical.
AI as the operating model
Stephen Hemsley said on Jul 16, 2026 that UnitedHealth Group sees AI as the operating infrastructure of the future and is reimagining virtually everything it does, with every function, HR, finance and legal included, being redesigned around AI. Hemsley told investors in Apr 2026 that the goal goes beyond productivity to how UnitedHealth Group organizes, operates and works, and that the pace is matched with prudence.
Internal tools become Optum Insight products
Sandeep Dadlani said on Jul 16, 2026 that about a third of UnitedHealth Group's 2026 AI investment goes to turning internal use cases into external products, such as the digital prior authorization tool sold under the Optum Real brand. UnitedHealth Group's board tied 20% of senior executives' 2026-2028 Performance Improvement Awards to reportable earnings from AI products sold outside the company (Apr 2026 proxy).
Several model and cloud partners
UnitedHealth Group spreads its AI work across vendors: Optum expanded Optum Real with Microsoft in Mar 2026 and with Google in Apr 2026, announced a partnership with Anthropic on Jul 13, 2026, and joined the Anthropic Partner Network by Oct 2026. Sandeep Dadlani described the Microsoft and Google agreements as go-to-market partnerships across products (Oct 2026).
Clinicians keep coverage decisions
Tim Noel said on Jul 16, 2026 that a clinician will make any decision not to authorize a service at UnitedHealthcare and that its contact centers will become more efficient but never be fully automated. UnitedHealth Group states that AI is not used to make clinical denials and that a qualified medical professional reviews every clinical denial.
- Deployment model
- UnitedHealth Group runs AI as a shared platform with delivery in the businesses. United AI Studio and enterprise AI delivery sit under Optum Chief AI Officer Shobhit Varshney (Jul 2026), while UnitedHealthcare, Optum Health and Optum Rx deploy AI in their own claims, service, clinical and pharmacy operations, and Optum Insight sells AI products to other payers and providers. Each UnitedHealth Group business unit reviews its AI portfolio monthly and CIOs meet quarterly to monitor AI use across the enterprise (Fortune, May 2025), and Julie Durham said in May 2026 that UnitedHealth Group holds a monthly AI business review with Stephen Hemsley.
- Model providers
- UnitedHealth Group gives employees large language models from the major AI hyperscalers and small language models it trained on proprietary data through United AI Studio (Fortune, May 2025), and Sandeep Dadlani put the number of large language models available to staff at 117 in Jun 2026 (Bloomberg), without naming them. Optum announced a partnership with Anthropic on Jul 13, 2026 to use Claude to reduce administrative work, without naming use cases. Microsoft's Dragon Copilot and Microsoft Foundry support clinical documentation and Optum Real, and Google's models and infrastructure run payer-facing Optum Real work such as benefits, prior authorization and claim status, according to Constellation Research (Jul 2026). Julie Durham said in May 2026 that tools such as Claude Code, Cowork and Codex mark the start of giving frontline staff frontier AI that had mostly been limited to executives and IT project teams.
- Data and infrastructure
- UnitedHealth Group draws on data from 48.3 million UnitedHealthcare members (Q3 2026) and the patients, pharmacy members and payer clients Optum serves. Optum's Microsoft agreement for Optum Real combines Microsoft Azure, Dragon Copilot and Microsoft Foundry (Mar 2026), and Google Cloud supports Optum Real's payer workflows (Apr 2026). Constellation Research reported in Jul 2026 that AWS provides Amazon Connect for UnitedHealth Group's conversational and agent-assist systems, with Amazon Bedrock, SageMaker and AgentCore also part of the relationship, and that Databricks is the data foundation for claims, embeddings, vector search and agentic workflows, citing Optum job listings for the Amazon Connect migration and the Databricks work. Optum Health moved nearly 100% of its employed provider groups onto three electronic medical record systems, down from 18, which Patrick Conway said in Jan 2026 lets it adopt AI faster. UnitedHealth Group has not said whether it runs models in its own data centers.

Enterprise AI platform run from Optum, AI products sold through Optum Insight, governance under the Chief Legal Officer
UnitedHealth Group has no chief AI officer at the parent level: Shobhit Varshney, who joined in Jul 2026, holds the title at Optum but is responsible for how AI is built, governed and scaled across Optum and UnitedHealth Group, according to CDO Magazine, and Ujjwal Ratan became chief AI officer of Optum Insight in Sep 2026. Sandeep Dadlani, the former chief digital and technology officer, has run Optum Insight since Sep 2025, Michael Pencina has been chief AI scientist since Oct 2025, and the Chief Legal Officer has owned AI governance since 2024. UnitedHealthcare and Optum's businesses deploy AI in their own operations under their CEOs, Tim Noel and Patrick Conway.
UnitedHealth Group's AI workforce sits mostly in Optum's technology organization, which Julie Durham put at about 40,000 engineers in May 2026. Sandeep Dadlani put UnitedHealth Group's AI engineers at 20,000 in Jun 2026 (Bloomberg), while an Optum Rx page cited more than 10,000 AI expert engineers in Mar 2026; the two figures may count different groups. About 20,000 UnitedHealth Group engineers had accepted AI-written code by May 2025, and more than 10,000 employees enrolled in an advanced AI course launched in Mar 2025.

Spend and ROI
AI investment
UnitedHealth Group plans to invest nearly $1.5 billion in AI-related initiatives in 2026 and at least as much in 2027, Stephen Hemsley said in Jan 2026 and repeated in Apr 2026; UnitedHealthcare's Mar 2026 Avery release put the 2026 figure at $1.6 billion, and Bloomberg reported in Jun 2026 that UnitedHealth Group plans to invest $3 billion in AI over 2026 and 2027. Sandeep Dadlani said about a third of the 2026 amount goes to Optum Insight's push to become AI-first and to turn internal tools into products (Apr and Jul 2026).
Reported ROI
UnitedHealthcare expects operating cost reductions of nearly $1 billion in 2026, many of them AI-enabled, Tim Noel said on Jan 27, 2026, without saying how much comes from AI. Sandeep Dadlani said on the Apr 21, 2026 call that UnitedHealth Group expects a 2:1 return on its AI investments over the next few years, with many projects paying back within 12 to 18 months, according to a GuruFocus summary. By Jun 19, 2026, UnitedHealth Group executives told Bloomberg they were already seeing a 2-to-1 return, without saying how the ratio is calculated or which costs and gains it counts. Dadlani said in Jul 2026 that part of Optum Insight's ahead-of-plan second quarter came from early AI efficiency gains. UnitedHealth Group had not published a realized dollar figure for AI savings as of Oct 2026.
Operating costs
UnitedHealth Group guided to a 2026 operating cost ratio of 12.8%, plus or minus 0.5 points, citing productivity work and the early benefits of AI investment (Jan 2026), then said in Jul 2026 the ratio would land at the higher end because of investment in people, communities and AI. UnitedHealth Group's operating cost ratio was 12.8% in the third quarter of 2026, down from 13.5% a year earlier; the Oct 13, 2026 earnings release credited realized operating efficiencies, partly offset by investment in technology, processes and people, and listed operational agentification among the investments, without isolating AI's effect.
Productivity markers
UnitedHealth Group reports task-level gains from AI: 33% less chart review time and 26% shorter appeal cycles (Oct 2026), nurse care managers working 40% faster and 96% first-pass approval in digital prior authorization (Jul 2026), and Optum Rx credited digital and AI self-service with 25% lower contact center volume (Apr 2026). The Jul 2026 call transcript also gives 69,000 for the digital prior authorization product, attached both to outside customers' authorizations and to administrative hours saved. UnitedHealth Group has not converted any of these into cost or headcount figures.
What AI itself costs
UnitedHealth Group has not broken out spending on models, tokens or cloud capacity within its AI budget. Julie Durham warned in May 2026 that uncontrolled token use is expensive and must be monitored, and said some engineers working through the night with AI tools raise concerns about burnout and token costs.
Headcount
UnitedHealth Group had more than 390,000 employees at Dec 31, 2025, nearly 165,000 of them clinical professionals. UnitedHealth Group took a $2.5 billion charge in the fourth quarter of 2025 for restructuring that included workforce reductions, which it did not attribute to AI. Tim Noel said in Jul 2026 that UnitedHealthcare will reinvest some AI savings in higher-touch service and that its contact centers will never be fully automated.
Executive pay tied to AI
UnitedHealth Group's Performance Improvement Awards for senior executives, which vest on 2026-2028 results, weight AI leadership at 20%, measured as reportable earnings from AI commercial products, alongside cost reduction and cybersecurity measures (Apr 2026 proxy).
Governance and risk
Board oversight
UnitedHealth Group's Audit and Finance Committee oversees the AI governance framework and the controls on legal, regulatory, privacy, security and financial risks from AI and automated decision tools, and the Health and Clinical Practice Policies Committee oversees the responsible and ethical use of AI in clinical, coverage and operational processes, according to the 2026 proxy statement.
Legal owns AI governance
UnitedHealth Group assigned ownership of AI governance to the Chief Legal Officer and the Legal and Compliance teams in 2024, separating them from the teams that build, deploy and monitor AI. A Responsible AI Program Office keeps the inventory of AI solutions, and the AI Review Board of technologists, clinicians, medical ethicists, data scientists, security, privacy and legal staff and member advocates can approve AI models or require their removal from production (2026 proxy).
Review cadence
UnitedHealth Group's responsible AI board reviewed hundreds of AI use cases each month before they reached production, each business unit reviewed its AI portfolio monthly, and CIOs met quarterly to monitor AI use across the enterprise, Sandeep Dadlani told Fortune in May 2025. Julie Durham said in May 2026 that Stephen Hemsley holds a monthly AI business review.
Humans make denials
UnitedHealth Group states that AI is not used to make clinical denials and that a qualified medical professional reviews every clinical denial. Optum sends any prior authorization its AI does not approve to a person (Jul 2026), and its InterQual Auth Accelerator does not auto-deny cases (Feb 2026).
Litigation and scrutiny
UnitedHealth Group faces a class action alleging that a legacy Optum algorithm drove improper Medicare Advantage post-acute care denials, which Optum denies, saying physicians make medical necessity decisions (HIT Consultant, Jul 2026). A GuruFocus report in Jun 2026 cited a federal inspector general report linking the naviHealth algorithm to higher denial rates that were almost always overturned on appeal.
Disclosed risks
UnitedHealth Group's 2025 Form 10-K warns that inaccurate or poorly protected data in systems that use AI could cause product failures, pricing errors and regulatory sanctions, that generative AI will play a growing role in its systems and products, and that fast-changing federal, state and international AI rules could impose new compliance requirements.

Quick answers
- How does UnitedHealth Group deploy AI?
- UnitedHealth Group deploys AI through United AI Studio, an in-house platform that gives its teams hyperscaler models and small language models trained on company data, and says more than 1,000 AI tools run in production across UnitedHealthcare and Optum.
- What does UnitedHealth Group use AI for?
- UnitedHealth Group says more than 1,000 AI tools run in production (Jan 2026), in member service, claims, prior authorization, appeals, clinical documentation, pharmacy and the products Optum Insight sells to other payers and providers. UnitedHealth Group has published no business return for any single use case; executives report operating and task measures instead, such as 25% lower Optum Rx contact center volume credited to digital and AI self-service (Apr 2026), 33% less chart review time and 26% shorter appeal cycles (Oct 2026). At the firm level, UnitedHealthcare expects nearly $1 billion of 2026 operating cost reductions, many AI-enabled, and executives told Bloomberg in Jun 2026 that they see a 2-to-1 return on AI spending, without saying how it is measured.
